Developed and implemented the Stroke Rehabilitation Care Pathway, improving post-stroke management, continuity of care, and patient outcomes. Established the Long-Term Care Service Pathway, leading the development, expansion, and integration of services, including the creation of a dedicated isolation unit to ensure patient safety and quality standards.
